This collection includes many forms from a wide variety of creators... some known and some not. If you find your form in this collection but I haven't credited you, please let me know! Simply a result of an admiring QuickMail user sending me your form to pass along. And if you find a form in here that you sent me personally, but your name doesn't appear here... I humbly apologize. It's just a matter of early senility. :-)

Well, you asked...
Yes, one of those will look familiar... the "QMonline 1.1" form is very similar to the "QMonline" form that was part of the first MHA Forms Collection. I have revised the form to make it resizable and zoomable.
Some of these, of course, will need some customization before you use them. For example, "From the Desk of..." (a great form from Michael Rose) has my name on it! And "Lead" is a sales lead transmittal form that says "BAKA" on it, but that can be fixed.
Please do send me some e-mail or get in touch some other way if you like these forms, so that I'll know that I should continue putting together these collections.
